Not neccessarily rare. Any disease that would slowly disable the Mede Emperor, leaving him unfit to rule, could be taken as proof that he was not suitable or fit to rule any longer. Ambitious courtiers might see an opportunity to shortcut the process if they knew he was on the way out. Definitely grounds for fear on his part!
